29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Job site, 2011 Cemetery Road, New Alexandria, PA: On or about April 10, 2013, during work inside of a well pit, the employer did not ensure that fall protections systems/devices or covers were provided around the opening to the pit that was determined to be approximately 5 feet by 3 feet in size, exposing employees to fall hazards of up to approximately 40 feet. A cover was provided for the opening to the well pit, but was kept raised while someone was in it doing work, with a person acting as an attendant at the top.

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length was not extended over its support more than 18 inches (46 cm), and was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, and guardrails were not provided to block employee access to the cantilevered end: a) Job site, 2011 Cemetery Road, New Alexandria, PA: On or about April 10, 2013, during construction activities, the employer did not ensure that the ends of scaffolding planks were prevented from cantilevering or that other actions were taken to prevent entry onto these sections of scaffolding. Scaffold planks were measured to hang over the ends of scaffolding by up to approximately 19 inches and and were not secured against cantilevering, nor were the ends of the scaffold blocked off to prevent someone from entering into the area.

Concerns noted included, but were not limited to, ends of scaffolding planks were not prevented from cantilevering/other actions were not taken to prevent entry onto these sections of scaffolding and wood planks had long cracks in them.
