29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)  Northeast Corner of Mezzanine, on or about April 10, 2013, and at times prior thereto, the controlling employer, Headwaters Construction Company did not ensure that its sub-contractor protected their employees while working on ventilation system and exposed to a fall hazard of approximately 8 feet from the Mezzanine to the concrete below.       (b)  Southeast side of Mezzanine, on or about April 10, 2013, and at times prior thereto, the controlling employer, Headwaters Construction Company did not ensure that its sub-contractor protected their employees while working on ventilation system and exposed to a fall hazard of approximately 8 feet from the Mezzanine to the concrete below.       Note:  Abatement documentation is required for this item, but Abatement certification is NOT required for this item.   Corrected the day of the inspection and certified by the sub-contractor.       Note:  The controlling employer, Headwaters Construction Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 589038, citation number 1, item number 1 and was affirmed as a final order on August 9, 2012 with respect to a workplace located at 333 West 6th South, Rexburg, Idaho 83440.    Note:  The controlling employer, Headwaters Construction Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 311575153, citation number 1, item number 2a and was affirmed as a final order on May 23, 2008 with respect to a workplace located at 138  N. Main Street, Victor, Idaho 83455.
