29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels: a. Site - On or about April 9, 2013, the HVAC duct shaft floor openings on the second and third floors were not guarded to prevent trash and other debris from falling through to the lower levels.
