Yard - Southeast Section, Dry Dock #1, Hull # 91: On or about 04/18/2013 the stair tower scaffold was not made safe in the following instances: (a) The a bolt securing the first level landing platform was missing the retaining nut. (b) The guard rail was missing the top rail and mid rail at the top of the stairs.
