29 CFR 1910.134(d)(1)(iii): When the employer was unable to identify or reasonably estimate the employee exposure, the employer did not consider the atmosphere to be IDLH: (Construction Reference 1926.103) (a) Location - 200 Shattuck Way, Newington, NH, inside water tank: On or about August 8, 2013, the employer did not the employer did not determine employees exposure to carbon monoxide and did not adequately determine employees exposure to oxygen prior to working inside the water tank. Employees used tight fitting face-piece type air purifying respirators while entering and exiting the tank where the atmosphere was potentially immediately dangerous to life and health (IDLH). Employees also used Bullard 88 VX sandblasting helmets which are not allowed by the manufacturer nor NIOSH under the approval TC-19C-293 for use in IDLH conditions. IDLH conditions could have been oxygen deficiency or elevated levels of carbon monoxide gas.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: (Construction Reference 1926.103) (a) Location - 200 Shattuck Way, Newington, NH, inside water tank: On or about August 8, 2013, Bullard 88 VX Series Airline respirators (sandblasting helmets) were left inside a water tank after sandblasting without being properly cleaned and stored to prevent dust contamination to the respirator.

Damaged hoses that were used to supply air to the helmets had been repaired. This practice is not allowed by the manufacturer nor NIOSH under the approval TC-19C-293.
