29 CFR 1926.250(b)(5): Materials in excess of supplies needed for immediate operations were stored on scaffold(s) or runway(s):      a) Oxford Mills, 100 West Oxford Street, Philadelphia, PA, south end-  Employees had the Oxygen/Acetylene cylinders for torch burning operations on the platform of the Genie aerial lift.  The cylinders far exceeded what was needed for the immediate operations of burning small unused pipes and conduits.  The employees had no means to escape the burning rig should there be a problem with a regulator, connection or hose that could flash in close proximity to the burning operation.  Observed on or about April 5, 2013.

29 CFR 1926.350(b)(1): Compressed gas cylinder(s) were not kept far enough away from the actual welding or cutting operation so that sparks, hot sag, or flame would not reach them:    a) Oxford Mills, 100 West Oxford Street, Philadelphia, PA, south end-  Employees had the compressed gas cylinders on the platform of the Genie aerial lift in close proximity to the sparks and hot slag generated by burning the old pipe and conduit penetrations out of the exterior walls.  Observed on April 5, 2013.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:       a) Oxford Mills, 100 West Oxford Street, Philadelphia, PA-  The temporary electrical outlets throughout the site consisted of two 4 inch square electrical junction boxes with knock outs screwed to 2 x 4 studs mounted in 5 gallon plastic buckets which is not in accordance with the junction box's listing and labeling.  Observed on or about April 5, 2013.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:      a)  Oxford Mills, 100 West Oxford Street, Philadelphia, PA-  The temporary electrical boxes throughout the site were located on 2x4 lumber mounted in five gallon buckets a short distance off the floors.  The connection at each location for the receptacles was made outside the electrical boxes and the conductors were only guarded against contact by electrical tape and wire nuts.  Contact or strain by tools, materials or debris during demolition activities could cause exposure of energized conductors.  Observed on or about April 5, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.     a)  Oxford Mills, 100 West Oxford Street, Philadelphia, PA, Second and Third Floors-  Employees were exposed to fall hazards of twenty to thirty feet to the level below where exterior windows had been removed from the second and third floors of the building.  The sill heights on the windows ranged from 24 to 26 inches.  There were two unprotected windows on each floor and one window with a chute with no rail above the wheel barrel ramp to the chute.  Observed on or about April 5 and April 8, 2013.

29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:     a)  Oxford Mills, 100 West Oxford Street, Philadelphia, PA, North End Stair Tower-  Employees were exposed to fall hazards in that the ramp provided to access the third floor from the temporary scaffold stairs did not have an adequate guard rail.  The planks were approximately 30 feet above the lower level and the top rail height was too low sloping from 27 inches to 30 inches.  Observed on or about April 5, 2013.        Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
