29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous:  (a) On or about April 02, 2013, and at times prior thereto, employees were exposed to an electrical hazard by using a faulty extension cord on the job site. The extension cord was missing the grounding prong on the male end eliminating the ground safety circuit.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:      (a) On or about April 02, 2013, and at times prior thereto, employees were not wearing fall protection equipment and were exposed to a fall hazard of approximately 13 feet to the ground below while replacing a residential roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders was not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):   (a) On or about April 02, 2013, and at times prior thereto, employees were exposed to a fall hazard of approximately nine feet to the ground below while using a thirteen foot non-self-supporting ladder set up at an excessive angle to gain access to the lower level roof of the house on the southwest side. The base of the ladder was set up four and half feet from the supporting wall, and was approximately 9 feet to the top support/eave.
