(b) Garage: On or about October 23, 2013, and at times prior, employees were exposed to a fall of approximately fourteen feet four inches to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.

(b) Garage: On or about October 23, 2013, and at times prior, employees were exposed to a fall of approximately fourteen feet four inches to the ground below while installing composite asphalt shingles on a 6:12 pitch steep slope roof. THE DOMINGUEZ FAMILY ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 901542, CITATION 01, ITEM NUMBER 003 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 13, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 587 1ST STREET WEST, BILLINGS, MONTANA 59101
