29 CFR 1910.145(f)(5): Danger tags were not used where an immediate hazard presented a threat of death or serious injury to employees: a) Lesko Enterprises, Inc. / 21 Euclid Street Albion, PA: On or about April 19, 2013, a grinder used to grind out of spec material contained a damaged and missing section of guarding and damaged flexible cord. The machine was not tagged out and/or rendered safe through other equally effective methods such as locking it out or moving the machine to a secure location to prevent its use.

The flexible cords protective out layer was pulled from around the electrical wiring at the power supply box on the machine. Functional strain relief devices were not installed in/around the area where the flexible cord entered the machines power supply box.
