29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.      On April 19, 2013, at 829 NW 79th Ter, Kansas City, Missouri Employees were exposed to a fall from a height hazard in that they were working on a roof approximately 25 feet above ground level without using an approved form of fall protection, and were not trained, or instructed, to do so.  The employer had no certification of training taking place.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On April 19, 2013, at the jobsite located at 829 NW 79th Ter in Kansas City MO, employees were exposed to a fall hazard in that they were working on a steep roof approximately 25 feet above ground level that required them to descend to a lower level, greater than 19 inches,  without the use of a ramp, runway, sloped embankment, hoist or ladder.
