Construction personnel were exposed to laceration hand injury when using the exit stairways.        Location: 404 Park Ave S, New York, N.Y;  at the 6th floor of the construction jobsite. 29 CFR 1926.502(i)(3): All floor hole covers were not secured when installed so as to prevent accidental displacement by wind, equipment, or employees:      a)  At the construction site: Floor hole covers were not secured on floors 4, 5, 6 of the kitchen exhaust shaft.  There were cleats on the underside of the covers to keep them in place but the covers were not nailed (shot-nailed) down.  A floor hole cover on the 6th floor shifted out of place, exposing employees to fall hazard.      Location: 404 Park Ave S, New York, N.Y;  at the 6th floor of the construction jobsite.
