29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations were not determined by safe and acceptable means: (a) On or about April 22, 2013, an underground gas line was struck and pierced by an operator operating a mini excavator in an area where the utility was known to be present due to recent utility markouts which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
