29 CFR 1910.134(c): Respiratory protection program. This paragraph requires the employer to develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use. Employees were exposed to respiratory hazards in that the written respiratory protection program did not contain worksite specific procedures such as when to use respirators during which job activity.

29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve. Employees were exposed to inhalation hazards including Dyhard MI-FF in that respirators were not properly cleaned and stored to prevent hazardous dust contamination. In the hot melt film line room 2 respirators were not stored properly as they had been left hanging on the equipment.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. Employees were exposed to inhalation hazards such as N,N-Dimethylformamide and Dyhard MI-FF in that fit tests were not provided for employees wearing the 3M full face respirators. Employees were required to wear respirators when adding powders to mixing vessels and while cleaning the solvent treaters.
