29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: At the north side of the shop on April 24, 2013, the left work rest on the Ryobi 6 inch bench grinder, number BGH616 was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The distance from the wheel to the edge of the work rest was about 1 inch.

The cylinder was not connected for use.
