29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. At the passageway located at the back side of the mezzanine on April 25, 2013, boxes of unknown food or drink supplies cluttered the passageway, and were not maintained in an orderly fashion. The boxes prevented employees from properly setting up the extension ladder to access the mezzanine. Employees in the vicinity or who ascend and descend from the extension ladder were exposed to potential trip and fall hazards.

29 CFR 1910.26(c)(3)(i): A ladder was not setup at the proper angle so the base distance from the vertical wall equaled one-fourth the working length of the ladder: At the back side of the mezzanine, on April 25, 2013, employees were exposed to fall hazards of up to approximately 9 feet, while ascending and descending from a aluminum extension ladder to access the mezzanine. The distance from the base of the ladder to the base of mezzanine wall was 6 feet 3 inches.
