29 CFR 1910.102(a):  Employers did not ensure that the in-plant transfer, handling, storage, and use of acetylene in cylinders comply with the provisions of CGA Pamphlet G-1-2003 ("Acetylene") (Compressed Gas Association, Inc., 11th ed., 2003).          On or about April 24, 2013, the employer did not ensure that compressed gas cylinders in Bay 1 area were stored in accordance with the Compressed Gas Association Pamphlet G-1, section 4.2.5 and section 4.2.10.  The employer did not ensure that acetylene cylinders were not stored in close proximity to oxygen cylinders, exposing employees to fire, explosion, and struck-by hazards.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.        On or about April 24, 2013, the employer did not ensure the points of operation of equipment were guarded:    a.  An employee was in use of a Kysor Johnson HA-16 horizontal band saw with approximately 11 ½ inches of exposed blade at the point of operation and approximately 4 ½ inches of exposed blade at the right side of the point of operation.  The employee was cutting 3 inch wide material exposing the employee to amputation hazards.      b.  An employee was in use of a Hemsaw side winder auto-4ft with approximately 10 inches of exposed blade at the point of operation and approximately 11 inches of exposed blade at the left side of the point of operation.  The employee was cutting two separate 1 inch pieces of material exposing the employee to amputation hazards.      c.  An employee was in use of a Webb Roller without tripwires provided to shut the roller off in an emergency.
