Employees were exposed to amputation hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about October 14, 2015, and at times prior thereto, electrical equipment were observed being used not in accordance with instructions included in the listing or labeling, including but not limited to the following instances: 1) In the electrical department, electrical relocatable-power-taps (RPT) were used to provide power to at least four different pieces of electrical equipment and permanently mounted on a structure. 2) In the electrical and saw department areas, electrical four-plex outlet boxes were used laid on the ground and/or horizontal surfaces instead of mounting on a structure. Employees were exposed to fire and electrical hazards.
