29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a)  The Oaks Apartments, 215 Tudor Lane, Manchester, CT. - On or about 4-25-13 - Employee loading material on the Laddervator was exposed to head injury by not wearing head protection while hoisting material to the roof 19 to 20 feet in height.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    a)  The Oaks Apartments, 215 Tudor Lane, Manchester, CT. - On or about 4-25-13 - Employees were exposed to electrical shock and burn hazard from extension cords used to power portable hand saws which did not have a GFCI (ground fault circuit interruper) in the system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.    a)  The Oaks Apartments, 215 Tudor Lane, Manchester, CT. - On or about 4-25-13 - Employees were accessing the roof work area from an aluminum extension ladder that did not extend three feet above the eave of the roof exposing employee to a 16 to 17 foot fall hazard.

The M & M Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315555771, citation number 2, item number 1, and was affirmed as a final order on 8/28/11), with respect to a workplace located at 31 Todd Road, Wolcott, CT.
