29 CFR 1910.305(g)(1)(iv)(A) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) On or about April 24, 2013 the inspection revealed that an extension cord was being used as a substitue for fixed wiring at the catwalk of the chip loading spout. The hazard was located at 20 Marine Drive in Boardman, Oregon.

29 CFR 1910.305(g)(2)(ii) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced: (a) On or about April 24, 2014 the inspection revealed that the flexible cord providing power to the small portable winch at the chip loading catwalk had been spliced in a manner that did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced. The hazard was located at 20 Marine Drive in Boardman, OR.

29 CFR 1910.334(a)(2)(i) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) On or about April 24, 2013 the inspection revealed a flexible cord with damaged outer insulation that had not been identified and removed from service. The extension cord was located on the catwalk at the chip barge loading area and was available for use to provide power to a portable electric winch. (b) On or about April 24, 2013 the inspection revealed an extension cord with damaged outer insulation that was available for use by the chip dump pit.
