29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) Spout platform: The Baldor industrial motor did not have a cover over the electrical wiring connections. This exposed employees to potential electrical shock from accidental contact with the connections. (b) Spout platform: The splice point connection box for the Baldor industrial motor did not close exposing the electrical connections. This exposed employees to potential electrical shock from accidental contact with the connections.

29 CFR 1917.26(f): A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees work exposed them to the hazard of drowning: Dock: The employers life ring had only 65 feet of rope attached. This exposed employees to potential delays in getting to the life ring in the event they fell into the water.
