29 CFR 1910.219(c)(2): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location required:  Web Press #2 Line, Baldwin Stobb Stacker.  Stacker operators were exposed to an unguarded rotating horizontal shaft on the stacker when routinely pressing the cycle stop button during each bundling sequence.  There is no documentation or certification required for this item.

Stacker operators were exposed to unguarded inrunning nip points between belts and pulleys on the stacker when routinely pressing the cycle stop button during each bundling sequence. On or about April 19, 2013, maintenance employee(s) who performed voltage testing on energized components inside a 230v-480v machine panel were not qualified to peform testing work on electric circuits and equipment.   Abatement certification is required for this item in accordance with the requirements of 1903.19(c).   Abatement note:  Qualified persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment (PPE), insulating & shielding materials and insulated tools (see 1910.333(c)(2)).
