29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, is not legibly marked to indicate its purpose, nor located and arranged so the purpose is evident.  This violation was most recently observed on April 26, 2013 at the location of 32 Indutrial Court, Seekonk, MA when:  Electrical panels on the west side of the building were not legibly marked with the correct identity of its circuits.  To abate this violation the employer must ensure that each service feeder and branch circuit at its disconnecting means or overcurrent device is legibily marked to indicate its purpose, or locateed and arranged so the purpose is evident.   Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it has taken to ensure compliance including, but not limited to, program improvements, job hazard analysis, written plans, site inspections, training and other appropriate measures.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more are not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  This hazard was most recently observed on April 26, 2013, at 32 Industrial Court, Seekonk, MA:  Circuit breaker openings in the 240 volt electrical panel on west side of building were covered with duct tape or were left open.  To abate this violation the employer must ensure that live parts of electric equipment at 50 volts or more are guarded against accidental contact by use of approved cabinets/enclosures or by other remedies identified in 1910.303(g)(2)(i)(A)-(D).  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it has taken to ensure compliance including, but not limited to, program improvements, job hazard analysis, written plans, site inspections, training and other appropriate measures.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings are not effectively closed:  This violation was most recently observed on April 26, 2013 at 32 Industrial Court, Seekonk, MA:  A 240 volt and a 480 volt electrical panel on west side of building had open holes in their sides.  To abate this violation the employer must ensure that unused openings in electrical boxes, cabinets, and fittings are effectively closed.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it has taken to ensure compliance including, but not limited to, program improvements, job hazard analysis, written plans, site inspections, training and other appropriate measures.

The cover for one spindle end of the Hammond Machinery Builders polishing machine did not prevent access to the rotating spindle nut.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it has taken to ensure compliance including, but not limited to, program improvements, job hazard analysis, written plans, site inspections, training and other appropriate measures.
