29 CFR 1910.132(f)(1)(iv): Employee(s) required to use personal protective equipment by this section were not trained to know the limitations of the personal protective equipment: Tool Room: On, or about, April 25, 2013, an employee suffered a serious hand injury when his protective gloves became entagled in the rotating tool provided on an Alliant veritical milling machine, serial number 1858. In accordance with company policy, the employee was wearing cut resistant gloves to prevent lacerations from the sheet metal that he was fabricating.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Tool Room: On, or about, April 25, 2013, an employee suffered a serious hand injury when the protective glove he was wearing became entagled in the rotating cutting head of the Alliant vertical milling machine, serial number 1858. The employee had been assigned the task of fabricating a chute out of sheet metal and was attempting to machine the edge of the part. The part required numerous adjustments in the vice which created a potential laceration hazard and exposed the employee to the rotating cutter.
