29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about 12/07/2015 employees were exposed to amputation hazards. Employees' cradle straightener was not affectively guarded to prevent entry into the danger zone. Empire Industries, INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number, 902804 citation number 01, item number 01b and was affirmed as a final order on 06/20/2013, with respect to a workplace located at 180 Olcott Street, Manchester, CT 06040.
