29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: a) Construction site, West entrance barrel roof - On or about April 9, 2013, three employees were tied off to an improperly secured single point fall arrest system roof anchor which did not meet the manufacturer's recommendations to ensure the anchorage point could support its 5,000 pounds of designed arresting force.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Construction site, West entrance barrel roof - On or about April 9, 2013 Employees working on a roof utilizing a rope and rope grab system for a fall arrest system, worked with the system rigged in a manner with excessive slack that could allow them to free fall more than 6 feet. Eastern Roofing Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.502(d)(16)(iii), which was contained in OSHA Inspection Number 313000648, Citation Number 1, Item 1 with respect to a workplace at Bedford Rd., Bedford Hills, NY, 10507.
