29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. a) Southlake Office Park - Employees were removing shingles on a low-slope 3 in 12 pitch roof, 12 feet high from the peak and 8 feet and 8 inches high from the gutter of the roof without conventional fall protection. Employees were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a) Southlake Office Park - An employee was using a 16 foot Werner ladder to access and egress the upper level of the roof. The ladder did not extend at least 3 feet above the landing and/or tied to the building to prevent displacement.
