29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) Mike Foreman, Bowling Green, Ohio: On or about April 29, 2013, at a worksite located at 725 McKinley St., Bowling Green, Ohio, the employer did not ensure that each employee was protected from falls while engaged in roofing activities on a steep sloped residential type structure. Two employees were exposed to potential falls of up to 18.2 feet to the ground below, which could result in permanent disability or death. No additional abatement information is needed for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Mike Foreman, Bowling Green, Ohio: On or about April 29, 2013, at a worksite located at 725 McKinley St., Bowling Green, Ohio, the employer did not ensure that each employee had been trained in the recognition and control of fall hazards. Two employees exposed to falls up to 18.2 feet were not trained to use fall protection systems, which could result in permanent disability or death. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
