On or about September 7, 2022 at the address of 732 Jefferson St. Bowling Green, OH 43402: The employer did not ensure employees were protected from falls while performing roofing activities atop a two-story residential structure with a 4-12 roof pitch. Employees operating in this conditions were exposed to falls of 9ft - 18ft to the ground below.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a)	Mike Foreman DBA Foreman Roofing and Construction. On or about September 7, 2022, at the address of 732 Jefferson St. Bowling Green OH 43402: The employer did not ensure employees using aluminum ladder had their side rails extended at least three feet above the landing surface. Two employees performing roofing work activities were exposed to fall hazards while using ladder to access/exit roof on a two-story residential structure with a height of  9ft - 18ft from ground below.

On or about September 7, 2022, at the address of 732 Jefferson St. Bowling Green OH 43402:The employer did not provide a training program for each employee potentially exposed to fall hazards while using an aluminum extension ladder to access a two-story residential roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.

Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Foreman Roofing and Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1437798, citation number 1, item number 1a and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 3747 West Alexis Rd., Toledo, OH 43623.
