29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a) DECA Warehouse: On or about 3 May 2013 and at times prior thereto, a hazard assessment was not conducted to address employee exposure to hazardous chemicals such as the corrosive product, Pure Bright Germicidal Ultra Bleach, used to clean floors. The product contains sodium hypochlorite and sodium hydroxide.
