29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  (a) Commissary, Bldg 536, Yuma Proving Ground, AZ 85365:  On or about 3 May 2013, and at times prior thereto, a door in the Admin Section which could be mistaken for an exit was not marked with a sign or other designation identifying it as leading to the Warehouse, exposing employees and others to hazards.

29 CFR 1910.157(e)(3): The date of the annual maintenance check of portable fire extinguishers was not recorded:  (a) Commissary, Bldg 536, Yuma Proving Ground, AZ 85365: On or about 3 May 2013, and at times prior thereto, the portable fire extinguishers mounted throughout the establishment did not have tags indicating the date that the annual maintenance check was conducted and recorded, exposing the employees to hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) Commissary, Bldg 536, Yuma Proving Ground, AZ 85365:  On or about 3 May 2013, and at times prior thereto, the legends inside electrical panels E-2 and E-1 contained errors in marking as to indications of circuit locations and purposes, exposing employees to hazards.

29 CFR 1904.32(a)(2): The employer did not create an annual summary of injuries and illnesses recorded on the OSHA log:  (a) DeCA Commissary, Bldg 536, Yuma Proving Ground, AZ 85365: On or about 3 May 2013, when requested to provide a copy of the OSHA Form 300A Annual Summary of Injuries and Illnesses for Calendar Year 2012, the employer could not produce a copy or provide an original certified form for review.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  (a) Commissary, Bldg 536, Yuma Proving Ground, AZ 85365:  On or about 3 May 2013, and at times prior thereto, the employer did not document that a hazard assessment of the workplace which required personal protective equipment had been conducted, the date of the evaluation, the person certifying the evaluation, and tht the document was identified as the "certification" of the hazard assessment.
