29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:      (a)  Beauty Shop - Beautician uses Motion Regular Relaxer containing sodium hydroxide, a corrosive and the employer did not have an adequate eye wash in her work space on or about May 8, 2013.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    (a)  Maintenance Shop - Maintenance Assistants were voluntarily using 3m Tekk 8511 n95 particulate respirators while sanding dry wall and they had not been provided with the information in appendix d, on or about May 8, 2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:   (a)  Stoddard Baptist Nursing Home - Employees were exposed to bloodborne pathogens as part of their work activities and their employer did not review and/or update their exposure control plan since September 14, 2011, on or about May 1, 2013.                                                        Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
