Maintenance was not being performed because a defective safety latch was not replaced.

(b) 820 Highway 7 North in Holly Springs, MS - On or about April 30 to May 1, 2013, PVC dry blend had accumulated on MTI mixer, cooler, the scale, and on top of the 2nd floor control room. (c) 820 Highway 7 North in Holly Springs, MS - On or about April 30 to May 1, 2013, PVC dry blend had accumulated on the ducts from the material screening and the ducts for blowing the dry blend to the silos.
