29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   Employees engaged in maintenance activities in the following locations were exposed to struck-by and caught-between hazards at the following pieces of equipment in that machine guarding was not provided:   a)  Body Shop, Millwright bay, Powermatic drill press, serial number 8720V044, at column PP32, was not equipped with chip guarding   b)  Body Shop, Millwright bay, Wilton horizontal band saw, equipment # 09A88BANSAW, at column PP32, approximately 11 inches of the unused portion of the band saw blade was not guarded.   c)  Tool and Die, Millwright bay, LeBlond lathe, equipment # 09A9021305, was not equipped with a chuck guard.   d)  Tool and Die, Millwright bay, Wilton-strand drill press, equipment # 09A9040001, was not equipped with chip guarding.    e)  Tool and Die, Millwright bay, Carlton radial drill press, equipment # 09A9040002, was not equipped with chip guarding.   f)  Tool and Die, Millwright bay, Alliant milling machine, equipment # 09AE4F0002, was not equipped with chip guarding.   g)  General Assembly, Millwright bay, Sharp radial drill, equipment # 569980, was not equipped with chip guarding.   h)  General Assembly, Millwright bay, Powermatic drill press, equipment # 09A88990DP, was not equipped with chip guarding.

Employees engaged in maintenance activities at the following pieces of equipment at the following locations were exposed to amputation hazards in that point of operation guarding was not provided:  a)  Body Shop, Millwright bay, Piranha iron worker, equipment # 09AE43ME280, was found to have an unguarded punch end with an opening of approximately 4 inches.  b)  General Assembly, Millwright bay, Piranha iron worker, equipment # 569658, was found to have an unguarded punch end with an opening of approximately 2.5 inches.
