29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) First floor, new residential construction located at 19 Creekwater Overlook, Acworth, Ga.: Four employees were exposed to falls of approximately 10-feet 6-inches while using a Werner 8-foot step ladder to access the second floor working surface located at heights of approximately 10-feet 6-inches from the first floor. The ladder used to access the second floor did not extend at least 3 feet past the upper landing surface.
