29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects:   JOBSITE: 402 Enchanted Parkway, Ballwin, Missouri  63021, at this jobsite, on or about 28 November 2012, workers were observed removing shingles from the rooftop and hurling the debris without a shoot directly onto the ground exposing them to potential head injury from falling or flying objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE:  402 Enchanted Parkway, Ballwin, Missouri  63021, at this job site, on or about 28 November 2012, the employer did not ensure that workers were protected from fall hazards when working on the roof at the apartment complex.  The roof pitch was approximately more than 4:12.
