29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about May 6, 2013, four employees observed working on a 5:12 pitched roof had fall protection harnesses on but not tied down to the anchors. Employees were exposed to a fall hazard of approximately 10-feet 3-inches to the ground.
