On or about April 4, 2024, on the south side of the building, an employee was performing masonry work activities from the third tier of a scaffold that was greater than ten (10) feet to the ground below and was not in use of fall protection. b.) On or about April 8, 2024 and at times prior thereto, on the west side of the building, an employee was performing masonry work activities from the second tier of a scaffold that was greater than ten (10) feet to the ground below and was not in use of fall protection. c.) On or about April 12, 2024 and at times prior thereto, on the west side of the building, employees were performing masonry work activities from the third tier of a scaffold that was greater than ten (10) feet to the ground below and were not in use of fall protection.
