OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to fire/explosion hazards: Sulfone Area-On or about 12/14/2012, an explosion occurred at the 20 million BTU/hour Waste Heat Boiler #731. The boiler did not properly purge resulting in injuries. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not used: a)Sulfone Area-Employee(s) were exposed to burn hazards while thawing a sulfuric acid line with a 200 pound steam hose. The employer did not ensure that employees wore the appropriate personal protective equipment (ppe) for the body while steaming the plugged sulfuric acid line on or about March 11, 2013.
