29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about 8/13/15, the employers machine specific Lockout/Tagout procedure did not clearly and specifically outline specific procedural steps for shutting down, isolating, blocking and securing switches and valves necessary for the control of hazardous energy to centrifuge #LM-522, in that the procedure did not include steps for the placement of Lockout or Tagout devices on energy sources such as but not limited to, LG- 522.pump breaker, LCP-522 Variable Drive, #1 Slurry Feed Valve, #3 MCB Header Flush Valve, #4 Main MCB Housing Flush Valve, #6 Main MCB Seal Flush Valve, #8 MCB Back Flush Valve to FV-522, #9 MCB Back Flush Valve to FV-522B, #13 LM-522 Nitrogen Valve, #35 LD-500 Drop Chute Valve, and #27 LD-800 Drop Chute Air Line.

29 CFR 1910.147(c)(1): Energy control procedures did not include effective alternative protective measures to provide protection to workers: On or about 08/13/2015, during normal production operations the employer failed to ensure the "Drop-Chute" valve was maintained in a closed position during routine, repetitive, and integral servicing tasks performed at the #LM-522 centrifuge as a means of alternative protective measures which would provide effective protection to workers. The employer alleges that the personal protective equipment required at the time including full face mask with organic cartridge and silver suit provided effective alternative protective measures. NOTE: Since the alleged incident, Solvay has redesigned the "Drop-chute" and valve assembly is now large enough to remain closed during the task and has implemented procedure M14-05-001 with an effective date of December 3, 2015 to ensure alternative effective protection measures for workers as require by the Minor Servicing Exception at 20 CFR 1910.147(a)(ii)(2).
