29 CFR 1910.134(c)(1):     A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    On or about 05/02/2013, the employer had not developed a written respiratory protection program for employees who are required to wear a  3M 5000 half-mask respirator and a 3M 6900 Full Face respirator, while engaged in blending, mixing,  and transferring operations.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:   On or about 05/02/2013, an employee using the Stokes R machine to press out tablets for approximately six hours, was wearing a 3M 5000 half-mask respirator.  The employee had a beard;  however, he had not been fit tested nor had he received a medical evaluation.
