A 22-foot aluminum extension ladder that was located in the patio area, extending from ground level to the second story balcony, and the ladder's side rails did not extend at least 3 feet above the upper landing surface being accessed; Instance b.) A 24-foot aluminum extension ladder that was located in the patio area, extending from the second story balcony to the roof, and the ladders side rails did not extend at least 3 feet above the upper landing surface being accessed.
