29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Jobsite located at 15 Freedom Blvd. Coatesville, PA.: The employer did not ensure that employees utilized protective helmets at the jobsite while roofing material was being discarded over the side of the roof with employees working approximately 50 feet below on the ground level, thus exposing them to struck-by hazards.  Condition observed on or about August 3, 2022.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a) Jobsite located at 15 Freedom Blvd. Coatesville, PA.: Employees were exposed to struck-by hazards while materials were disposed approximately 40 feet from a residential roof onto the ground level without using safe means such as a chute.  Condition was observed on or about August 3, 2022.

Condition was observed on or about Aug 3, 2022.

Employees were working in close proximity to the unprotected sides and edges of the roof and exposed to falls of approximately 50-60 feet.  Condition was observed on or about August 3, 2022.

Workers were exposed to fall hazards in excess of approximately 6 feet to the ground level, when using an aluminum extension ladder with side rails not extend at least 3 feet above the surface landing.  Condition was observed on or about August 3, 2022.
