29 CFR 1910.132(d): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which would have necessitated the use of personal protective equipment: (a) An adequate hazard assessment of the demolition work was not done. The assessment did not identify the stainless steel that had been added and the hazards associated with the cutting and removal of the stainless including the potential for exposure to hexavalent chromium.
