29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.    Anchorage port; on or about 7 May 2013 and times prior thereto, electrical boxes were not effectively closed. Three instances were observed:    Instance (a) - located in the dock area, lasher building; an energized electrical outlet, on the north wall, beneath the pay phones was missing a cover plate.       Instance (b) - located in the maintenance shop, parts and receiving room; a cover plate to an energized electrical outlet, on the left side of the stairway was open.       Instance (c) - located in the maintenance shop, mechanics work station, adjacent to wash bay; a knockout plug was missing from the electrical outlet on the left side of the mechanics work bench.     These conditions exposed employees to electrical shock hazards.

Maintenance shop, mechanic's pit area; on or about 7 May 2013 and times prior thereto; a flexible cord, approximately 50-feet in length was permanently affixed to an intermodal storage container via rubber lined P type retention clips.
