29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Dept. of Air Force Davis Monthan AFB - Logistics Readiness Squadron - On or about May 6, 2013 and at times prior thereto the Machine Shop: a) The Toolmaster Milling Machine with Serial Number 5531D5V-110 did not have a guard to protect employees from the point of operation of the rotating cutting head. b) Jet Head Engine Lathe with Serial Number 907W2582W did not contain a guard to protect employees from the rotating parts and flying metal chips during operation. The U.S. Air Force was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 467640, notice number 1, item number 4 and was affirmed as a final order on March 18 2013, with respect to a workplace located at Arc Light Blvd. Bldg. 22007, Andersen AFB Yigo GU 96929. The U.S. Air Force was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 191327, notice number 1, item number 1 and was affirmed as a final order on May 22, 2012, with respect to a workplace located at 730 E. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. Dept. of Air Force Davis Monthan AFB - Logistics Readiness Squadron - On or about May 6, 2013 and at times prior thereto, the access to and/or working space around electrical equipment was not maintained to permit ready and safe operation and maintenance of the equipment in the Allied Trades Welding Shop. Welding equipment inhibited access to 5 electrical disconnects boxes in the event of an emergency. The U.S. Air Force was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 467640, notice number 1, item number 5 and was affirmed as a final order on March 18, 2013, with respect to a workplace located at Anderson AFB, Bldg. 22007, HVAC Arc Light Blvd, Andersen AFB Yigo GU 96929.
