29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Public Side - Employees used a Cascade Slip Sheet material handling attachment serial number 35E-QB-168-03300P with a Komatsu 25 PIT serial number 582419A to transfer loose cargo from the floor onto pallets. The employer did not ensure that the truck was marked to identify the attachment and show the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered, on or about May 9, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
