BWP Hardwoods, Inc., 12942 Route 322, Brookville, PA 15825,-Various production areas- on or about May 8th, 2013, and times prior thereto: On 11/19/2012, when three company employees experienced a standard threshold shift, the Company failed to conduct refitting of and retraining on hearing protectors with these employees. Additionally, the Company did not determine if the employee required hearing protection with greater attenuation.

BWP Hardwoods, Inc., 12942 Route 322, Brookville, PA 15825, on or about May 8th, 2013, and times prior thereto: Company employees in the Resaw and Grading area were exposed to noise levels at or above 85 decibels and were included in the Company's Hearing Conservation Program. Company employees who were included in the Company's Hearing Conservation Program were not provided with annual training.
