Employees were not using fall protection while working on the steep roof. The distance between the steep roof and the ground was approximately 26 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about May 6, 2013, at the jobsite located at 6360 North London Ave in Kansas City MO: Employer failed to ensure that portable ladders were extended at least 3 feet above the eve of the roof. Employees that were ascending and descending the steep roof structure were exposed to fall hazards. The distance between the eve of the steep roof and the ground was approximately 26 feet.
