29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about December 5, 2017, employees were observed working from a shop made steel personnel lifting platform mounted to a forklift truck and elevated to a height approximately 25 feet. The personnel lifting platform was observed not properly secured to the lifting carriage and/or forks of the forklift truck. Employees were exposed to fall hazards.

The personnel lifting platform was not designed and used in accordance to American National Standards Institute B56.6-2011. Employees were exposed to fall hazards.
