On or about May 9, 2013, at the above addressed job site, employees were exposed to a fall hazard while installing tar paper on a roof without using a means of conventional fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. On or about May 9, 2013 at the above addressed job site, employees who had not been provided training to recognize fall hazards were installing tar paper on a roof without a means of conventional fall protection.
